If your usual approach to maintaining your weight has suddenly stopped working — the same meals, the same activity level, but the scale is moving in the wrong direction or your body composition is shifting — and you're in your 40s, there's a good chance the problem isn't what you're doing. It's what your hormones are doing.
Perimenopause weight changes are one of the most frustrating things I see in nutrition consultations, partly because they're physiological rather than behavioural, and partly because the advice that gets offered is so often generic — "move more, eat less" — without addressing what's actually happening underneath.
Why Your Body Responds Differently Now
Estrogen plays a role in where your body tends to store fat. As estrogen levels begin to fluctuate and decline during perimenopause, the pattern of fat storage often shifts from the hips and thighs toward the abdominal area. This isn't about willpower — it's a documented hormonal response that affects most women during this transition.
At the same time:
Insulin sensitivity decreases. Fluctuating estrogen affects how your cells respond to insulin, which means the same carbohydrate load that felt fine at 35 may trigger more of a blood sugar response at 45. More on this in our blood sugar support guide.
Cortisol has more impact. As ovarian hormone production declines, the adrenal glands pick up some of the slack — but they also produce cortisol. Chronic stress during this window directly contributes to both abdominal fat storage and carb cravings.
Muscle mass starts to decline. Estrogen also supports muscle protein synthesis. Without intentional resistance training, the gradual loss of muscle during perimenopause reduces your resting metabolic rate — which means the same caloric intake produces different results.
What Actually Works During Perimenopause
The strategies that worked in your 20s and 30s may need updating — not because you need to work harder, but because the levers have changed.
Protein becomes more important, not less. Most of the women I work with are under-eating protein relative to what their muscle maintenance actually requires. For women in perimenopause, 1.2-1.6 grams per kilogram of body weight is a meaningful target — and spreading it across meals (rather than saving it all for dinner) matters.
Resistance training is non-negotiable. Cardio alone is not sufficient if your goal includes maintaining metabolic rate and body composition through this transition. Two to three days of progressive resistance training per week is the most evidence-supported strategy we have.
Blood sugar stability matters more than total calories. Many perimenopausal women find that reducing the glycemic load of their diet — without dramatically reducing overall intake — makes a bigger difference than calorie restriction. Prioritizing fibre, pairing carbohydrates with protein or fat, and reducing refined carbohydrates is usually more sustainable and more effective than a deficit-based approach.
Sleep is not optional. Poor sleep elevates cortisol, increases insulin resistance, and drives cravings for high-sugar foods — three mechanisms that all work against you during perimenopause. If sleep is disrupted (which it commonly is), addressing it is part of the weight management conversation, not separate from it.
